After booking an appointment, we will email you a link to an Intake form.
- If you would like us to direct bill to your extended benefit plan, complete the Insurance Information section of the Intake form

Paper copies are available at the clinic, if needed
- If you’re planning to fill out the paper copy, we recommend you arrive a little earlier to fill it out before your appointment
- We’ll submit your claim during or upon completion of your appointment

Claims are submitted through a secure online portal
- The submission of your appointment will be dependant on your provider’s platform being available and working. From time to time, a variety of provider platforms will undergo updates or be offline. In this situation we’re unable to submit your claim
- In the event of the provider platforms being unavailable, you’ll need to pay for your treatment in full, and submit your invoice to your provider for reimbursement
- We receive confirmation of the amount covered by your benefit provider
- If applicable, we bill you for any amount not covered by your benefits

Sometimes claims are subject to additional review before your benefits provider will determine the amount to be covered
- When this happens, we bill for the full appointment fee and you will need to submit your receipt to your benefit provider to receive compensation

At this time, we can only do direct billing to your primary extended benefits plan.
We do our best to work with as many insurance companies we can. Due to a variety of reasons, we do not direct bill to all insurance plans.
At your appointment, we will provide you with a detailed receipt so you can submit it to your extended benefit plan provider to receive compensation.
